The Diabetic Annual Review programme is run by Dr Anne Hales in conjunction with Nurse Tor Godfrey and Margaret Connolly our Health Care Assistant. You will need to attend a local health centre for blood tests before being coming into the surgery for a double appointment – this will be coordinated by Margaret to ensure we find a time that suits you.
You annual review is a chance for ongoing education and support; it is also an opportunity to check a range of blood tests (inc. blood sugar, cholesterol, kidney function), urine tests, blood pressure and weight. We will also ensure that you have attended a retinal screening appointment and help you book an appointment if not. A medication review will be conducted to ensure that your current regime suits you and you will have ample opportunity to raise any concerns you might have about managing your condition in the long term.
The main aim behind the annual review programme is to prevent complications arising from, and to delay the progression of your diabetes.
