<p>A different speaker will attend each weekly session &#8211; some will be more discussion based, others will involve a bit of practical work and we hope that all will be productive and informative!</p>
<p>Refreshments will be provided, and we would encourage you to bring your newborn along.</p>

			<content:encoded><![CDATA[<p><strong></strong>Holidays need careful planning and, depending on the type of holiday,  length of stay and the destination, most will need some sort of  vaccination programme or an update of routine immunisations as  necessary.</p>
<p>Gap years and sabbaticals usually require more detailed immunisation  programmes. We would recommend you consult a practice nurse at least 6  weeks before your trip.</p>
<p>The surgery is an authorised yellow fever centre and provides a full  range of travel requirements. Any charges that apply for immunisations  compare favourably with other private travel clinics.</p>
<p>We would thoroughly recommend that all patients planning to travel  outside the EU visit <a href="http://www.fitfortravel.nhs.uk/home.aspx" target="_blank">the  NHS Fit for Travel site</a> – it contains a huge amount of useful  information on immunisations and healthcare abroad.</p>

			<content:encoded><![CDATA[<p>If you make a claim on your health insurance, your provider may require a  private claim form to be completed by your referring GP.</p>
<p>Some providers will need the form to be completed before they will  agree to fund your private treatment, others will accept forms after  treatment has taken place – please check your cover with the insurers  before seeing your specialist to avoid any confusion.</p>
<p>Your provider will usually issue you with the claim form directly. It  should then be forwarded, with a covering letter, to Julian Sula in our  administrative team.</p>
<p>Completion of these forms can be time consuming, usually involves  reading your entire medical record and carries a considerable  responsibility. Like most practices, we therefore charge an  administrative fee of £38 for this non-NHS work, payable as the forms  are dropped off.  This fee may or may not be covered by your provider.</p>

			<content:encoded><![CDATA[<p>Thurleigh Road Practice operates an ‘Open List’.  This means that we always welcome new patients to the practice provided they live within our catchment area.</p>
<p>To register at Thurleigh Road, first check that you live on one of the roads listed in the <a href="http://www.thurleighroadpractice.co.uk/nhs-practice/catchment-area/">Catchment Area</a>.</p>
<p>If you do, you will then need to bring proof of address in the form of a recent utility bill, tenancy agreement or similar, and photographic ID (passport or driver’s license), to reception.  Mobile phone bills and bank statements are not accepted as proof of address.</p>
<p>Each patient wishing to register must attend with proof of address and identification in person – unfortunately you cannot bring your spouse’s or children’s identification on their behalf.</p>
<p>A receptionist will then give you a set of New Patient Registration forms and book you a New Patient Health Check (NPHC) with one of our nurses.  You take them away, fill them out and bring the completed forms with you to the NPHC. <strong>Please note that you will also need to provide your NHS number on the registration form. We will not be able to register you if you fail to provide this.</strong></p>

<p>You annual review is a chance for ongoing education and support; it is also an opportunity to check a range of blood tests (inc. blood sugar, cholesterol, kidney function), urine tests, blood pressure and weight.  We will also ensure that you have attended a retinal screening appointment and help you book an appointment if not.  A medication review will be conducted to ensure that your current regime suits you and you will have ample opportunity to raise any concerns you might have about managing your condition in the long term.</p>
<p>The main aim behind the annual review programme is to prevent complications arising from, and to delay the progression of your diabetes.</p>
