Thurleigh Road Practice operates an ‘Open List’. This means that we always welcome new patients to the practice provided they live within our catchment area.
To register at Thurleigh Road, first check that you live on one of the roads listed in the Catchment Area.
If you do, you will then need to bring proof of address in the form of a recent utility bill, tenancy agreement or similar, and photographic ID (passport or driver’s license), to reception. Mobile phone bills and bank statements are not accepted as proof of address.
Each patient wishing to register must attend with proof of address and identification in person – unfortunately you cannot bring your spouse’s or children’s identification on their behalf.
A receptionist will then give you a set of New Patient Registration forms and book you a New Patient Health Check (NPHC) with one of our nurses. You take them away, fill them out and bring the completed forms with you to the NPHC. Please note that you will also need to provide your NHS number on the registration form. We will not be able to register you if you fail to provide this.
